Student Records

Use these forms to update personal information in UW-Stout's records.

Address Change Form

Use this form to change the permanent/home address that the university keeps on record. This does not apply to residence hall addresses, which are maintained through University Housing.

**International Students must go to the office of International Education or the Admissions office if they wish to change their address.

**If Employees of UW-Stout would like to change their address they will need to change it using their UW portal or contact Human Resources.

NOTE: This form does not update:

  • Your mailing address
  • Your hometown news address
  • The name and address that will appear on your diploma

To change these addresses, please follow this guide to update your address.

This form is only used for students.

If the student would like to suppress their personal information, such as phone numbers, e-mail addresses, and mailing addresses. Here are the steps to suppress information via Access Stout.

Name and Gender Change Form

If a student’s legal name and/or gender is changed this form allows them to change the name/gender on the school’s record. Please complete this form and upload/submit a copy of the legal documentation for the name and/or gender change (ex. marriage license, divorce decree, legal court paperwork). 

Race/Ethnic Code Change Form

Please complete this form.

This form allows students to change their race or ethnicity from what is currently in the system to a different race/ethnicity if the race/ethnicity on file in incorrect or the student now identifies differently. 

Registration Forms

Non-traditional and non-enrolled students may use these forms for registration and course-related purposes. 

Final Grade Report Request Form

A Final Grade Report is different than an official and unofficial transcript. A final grade report is a reflection of one terms grade and one final grade report may be requested at the end of each term. However, if your full academic history at UW-Stout is desired then you may request an unofficial or official transcript.

There is no charge for an official grade report. A stamped, self-addressed envelope is required for each semester/term requested. Only one request per envelope will be processed. Failure to complete this entire form may result in a delay in the issuance of your grade report.

Due to the Family Rights and Privacy Act of 1974, your signature is required for the release of a grade report. Be sure to sign this form after printing it.

Due to security reasons, final grade reports cannot be emailed to the student.

Stout Online Registration Form (Non-degree Students Only)

This form is to be used by non-traditional students only who are not seeking a degree with UW-Stout. If you are seeking a degree please use the official UW-System application to apply.

Summer and WinTerm Registration Form

New Students
Summer/WinTerm Session Registration Form must be completed online.  

Once the Registration form has been received and processed, an email will be sent to out with further instructions for completing the Student Account Contract, which is required before registration can begin.

Note: New students that wish to continue as degree-seeking students into the next term must go online to https://apply.wisconsin.edu/ and complete the UW System application for admission.

If a course you are registering for requires a prerequisite you completed at an institution other than UW-Stout, then you must forward an official transcript the instructor to confirm completion of the prerequisite.
